Rohbe's Survey Module
allows you to build supplier audit/surveys and then record the results online.
Survey Module features include:

Four Response Types - Numeric (1 to 10), Yes/No, Comments and Likert Scale

Interfaced with RSAS - Survey results passed directly to Supplier service scoring

Flexible Weighting - Weights assigned to both category and question
